Formal mental health assessment including diagnostic and treatment or accommodation recommendations
Service Description
Sometimes when we are impacted by mental health issues we need assistance from a therapist who has the expertise to accurately hear and document our story, then diagnose and make professional treatment recommendations. This type of assessment may be necessary for any documentation needs a client may have for medical, employment, disability or legal matters. We can provide copies of our formalized assessments to clients or those to whom clients sign to release information.
Cancellation Policy
While we make every effort to work with clients in times of crisis, any no shows or cancellations not made 24 hours in advance will incur a cancellation fee of $120 which will be charged to the client's payment method on file.
